The district nurse comes to your home to map out your care request.
In addition to personal care and nursing, we also provide individual support, benefits in kind, as well as for PGB or care to individuals.
Our caregivers help you with your daily physical care. They come to your home for that. This way you can continue to live independently and in your own environment for as long as possible.
Referral for home counseling can be done by your GP, your social worker, from youth health care, youth care, GGD, RIAGG, GGZ, MEE, addiction care, refugee work and other support agencies.
